I grant permission to use my submission in campaign materials, including advertising. By checking this box, you acknowledge and agree to the following:*

Content You Are Submitting. Your submission may include video recordings, photographs, written statements, testimonials, spoken words, and quotes, as well as your name, likeness, voice, and any personal information, including health or medical information, that you choose to share ("Content"). You should submit only information that you are comfortable having used publicly in campaign materials, including broadcast and digital advertising.
License You Are Granting. You grant YES on 38: Californians for Life-Saving Immunology Research and Cures ("the Campaign") a perpetual, irrevocable, royalty-free, fully paid-up, worldwide license to use, reproduce, edit, adapt, publish, translate, distribute, and display your Content, in whole or in part, in any medium or format now known or hereafter developed, for any Campaign-related purpose, including but not limited to documents, brochures, direct mail, electronic, digital, print, broadcast, satellite, and cable advertising, website and social media communications, earned-media submissions, and any other public-facing Campaign materials or communications.
Use of Your Name, Likeness, and Voice. The license above includes the right to use your name, photograph, likeness, and voice recording in Campaign materials, including paid advertising, consistent with California Civil Code § 3344. You understand that your image and voice may appear in television, radio, digital, and social media advertisements distributed throughout California and potentially nationwide.
Use of Health and Medical Information. If your submission describes a personal or family health condition, diagnosis, or medical experience, you understand and consent to the Campaign's use of that health-related information as part of your Content in the materials described above, including in advertising.
Your Representations. By checking the box above, you represent that: (a) you are at least 18 years of age; (b) you own or have the right to submit all Content you are providing, and the Content does not infringe the rights of any third party; (c) no other person whose image, voice, or personal information appears in your submission is identifiable without that person's consent having been obtained; and (d) you are voluntarily granting this license and have not been compensated for doing so.
No Obligation to Use. The Campaign is under no obligation to use your Content. If your Content is used, you are not entitled to compensation, royalties, or any other consideration beyond what is described in this license.
Withdrawal. You may request that the Campaign cease future use of your Content by contacting info@YESon38.com. However, withdrawal of consent does not affect uses already made or materials already in distribution at the time of your request.
